Service Description
The Agency shall provide directly or under arrangement Occupational Therapy by a qualified Occupational Therapist or Occupational Therapy Assistant under the supervision of qualified Clinical Supervisor and in accordance with the physician’s written orders. Occupational Therapy services shall include but not be limited to:
- The first visit is made within 48 hours to determine the immediate care and support needs of the client unless otherwise specified by the physician or requested by the client (with permission by the physician)
- Initial and ongoing Occupational Therapy assessments (OASIS assessments only when no other skilled discipline is involved) to identify the client’s level of functioning at appropriate points including discharge planning
- Assessment and reassessment of the client and evaluation of the client’s level of function by applying diagnostic and prognostic procedures
- Guiding the client in the use of therapeutic, creative and self-care activities for the purpose of improving function
- Instruction in adaptations necessary for performance of activities of daily living
- Design and instruction in the use of adaptive devices
- Development, implementation, review and renewal of the OASIS document and physician’s Plan of Care in cooperation with the physician and other members of the care team
- Advising and consulting with the family and other members of the care team, as applicable
- Development, implementation, review and revision of the plan of care and documentation of clinical and progress notes, summaries, and other documentation for the clinical record
- Participation in case conferences, staff meetings, and in-service programs as indicated
- Discharge Planning and evaluation outcomes of care
- Supervision of the Occupational Therapy Assistant
