Service Description
The Agency shall provide Physical Therapy services directly or under arrangement by qualified licensed Physical Therapists and Physical Therapy Assistants under the supervision of a qualified Clinical Supervisor and in accordance with the physicians’ Plan of Care. Physical therapy services shall include but not be limited to:
- The first visit is made within 48 hours to determine the immediate care and support needs of the client; and if admitting clinician for Medicare clients, to determine eligibility for the Medicare home health benefit, including homebound status; unless otherwise specified by the physician or requested by the client (with permission by the physician)
- Initial and ongoing comprehensive assessment to determine level of functioning at appropriate time points including planning for discharge
- Assessment and reassessment of the client and assistance in evaluating the client by administering muscle, nerve, joint and functional ability tests
- Treating clients to relieve pain, develop or restore function, and maintain maximum performance, utilizing teaching, exercise, massage, and applicable therapy treatments and modalities
- Directing and assisting clients in active and passive exercises, muscle reeducation, and functional training, activities of daily living, including transfer activities and prosthetic training
- Provision of therapy utilizing equipment such as ultraviolet and infrared lamps, low voltage generators, diathermy and ultrasonic machines
- Administering whirlpool and therapeutic baths, and applying moist packs
- Observing, recording and reporting to the physician the client’s reaction to treatment and any change in the client’s condition
- Instructing the client in the care and use of wheelchairs, braces, crutches, canes and prosthetic and orthotic devices
- Instructing other health team members, including Home Health Aides and family members, in the home program
- Development, implementation, review and renewal of the OASIS document and Plan of Care in cooperation with the physician and other members of the care team – Advising and consulting with the family and other members of the care team, as applicable
- Development, implementation, review and revision of the plan of care and documenting clinical and progress notes, summaries, and other clinical record information
- Participation in case conferences, staff meetings, and in-service programs as indicated
- Evaluating outcomes of care
- Supervision of the Physical Therapy Assistant and Home Health Aide
