Description
The Agency will provide Support/Service Coordination services to patients and consumers. Support/Services Coordination services will include educating, planning, advocacy, coordination and monitoring to assist consumers in gaining access to needed services. Support/Service Coordination services will ensure consumers are enrolled in the proper waiver service and other Medicare state plan services. Individual Service Plans (ISP) will be developed in accordance with physician requirements and consumer needs and submitted to HCSIS. Support/Service Coordination will constantly assess and re-assess the consumer’s level of care and the development, writing, and review of ISP to ensure the consumer is receiving the proper level of care.
