Service Description
The Agency shall provide speech therapy directly or under arrangement by a qualified Speech/Language Pathologist under the supervision of a qualified Clinical Supervisor. Speech Therapy services shall be performed in accordance with the physician’s Plan of Care and shall include but not be limited to:
- The first visit is made within 48 hours to determine the immediate care and support needs of the client; and if admitting clinician for Medicare clients, to determine eligibility for the Medicare home health benefit, including homebound status; unless otherwise specified by the physician or requested by the client (with permission by the physician)
- Initial and ongoing comprehensive assessment to determine level of functioning at appropriate time points including planning for discharge
- Assessment and reassessment of the client and evaluation and testing of clients with speech, hearing or language disorders
- Assisting the physician in determining and recommending appropriate speech as well as hearing services in the development and renewal of the physician’s Plan of Care
- Providing rehabilitative services for speech, hearing and language disorders
- Recording and reporting the client’s reaction to treatment and any changes in the client’s condition to the physician
- Instructing other health team and family members in the home rehabilitation program
- Development, implementation, review and renewal of the Plan of Care and OASIS document in cooperation with the physician and other members of the care team
- Advising and consulting with the family and other members of the care tam, as applicable
- Development, implementation, review and revision of the plan of care and documenting clinical and progress notes, summaries, and other clinical record information
- Participation in case conferences, staff meetings, discharge planning activities and in-service programs as indicated
- Evaluating outcomes of care
